How to change or cancel your appointment

If you have an outpatient clinic appointment booked at Bristol Eye Hospital but would like to change or cancel the date, please complete the form below.  This form sends information to a hospital e-mail address. We will always treat the information you send us with the strictest confidence and will not pass any information on to third parties.
