This section allows you to download your registration and triage
forms toprint and fill in. We hope that if you do this it
will allow you to get through reception quickly and speed up the
duration of your visit.
Please remember;
- If you have an appointment, please arrive 10 minutes before to
allow time to check-in (even if you have downloaded and completed
the forms).
- (Men) Please hold your urine for at least two hours before your
appointment.
- You can check our homepage or follow us on Twitter for updates
on the status of the walk-in sessions.
- We keep all data held in the clinic safe and secure. Please
take care in getting the completed forms to us.

All listed services are provided free of charge, you can just turn
up, or call us to arrange an appointment (no need to be
referred).
