Patient Affairs/Bereavement Services
Our Patient Affairs/Bereavement Services can be found in room
A409, BRI. Their role is:
- To arrange for the completion of the medical certificate (which
must be collected from Patient Affairs by the next of kin at an
agreed time).
- To be responsible for any personal effects and/or valuables to
be made available for collection by the next of kin.
- To provide help and advice to families regarding registering a
death, funeral arrangements and any other related issues (including
how to obtain financial help with funeral costs etc).

Medical Examiner Office
Following a bereavement, many families seek reassurance
that their loved one was properly cared for in hospital and want to
understand the circumstances surrounding their death.
The Medical Examiner service has been introduced in England and
Wales to increase understanding and peace of mind for families, as
well as improve learning and patient safety in hospital by
