Access/referrals
Referral should be by letter to BHOC Dietitians
(telephone referrals should be followed up in writing), giving the
following information.
- Patient's details - surname and forename; age
and sex; address; telephone number
- Clinical information - diagnosis; relevant
medication/ other information including relevant biochemistry;
dietary intervention required
- Details of referrer - name and signature of
GP; GP contact details
Referrals are prioritised according to the degree of dietetic
urgency. Further information about this is available on
request.
Key areas of practice
Clinical dietetics
Patients with a wide range of illness are treated by:
- assessing dietary needs
- designing an individual plan to meet these needs
- helping patients / carers to achieve any desired dietary
changes
- evaluating the effect of the changes
Education: Good Nutrition makes a
difference
We aim to increase awareness of the links between what we eat
and health by:
- helping individuals to make good food choices
- working with small groups to help develop knowledge and skills
in specific areas
- participating in health promotion campaigns
