University Hospitals Bristol NHS Foundation Trust has completed
the implementation phase of the electronic discharge summaries
(eDiS) project. The process is live on 45 wards in six
hospitals. More than 12,000 eDiS have been completed over the
last 10 months since the first ward went live in December 2009.
The Adult Discharge Summary template has been modified in
response to feedback from doctors within and outside the
Trust. The other templates available are Adult Day Case for
elective patients, Adult Self Discharge, Adult Short Leave and
Notification of Death. Two templates are available for
paediatrics, one for medical patients and one for surgical
patients. A formal process for reviewing the templates in the
future will be set up.
The Trust has been sharing information with North Bristol NHS
Trust (NBT) about how the eDiS project has been implemented, to
ensure that NBT can benefit from the good practice and lessons
learned from this project.
The Trust is working closely with the Avon IM&T Consortium
to ensure that GP practices have access to eDiS, and Order Comms in

Practice, Western College Family Practice and Whiteladies Health
