The Trust works closely with a number of organisations
across the region. A section 31 Partnership Agreement was concluded
in January 2003 between University Hospitals Bristol NHS Foundation
Trust and the following organisations:
- NHS Bristol
- North Bristol NHS Trust
- Bristol City Council Social Services
The Partnership Agreement binds the parties to work
to:ยท
- Set minimum acceptable thresholds for delayed transfers of
care
- Establish systems to monitor delayed transfers of
care
- Achieve a sustained reduction in delayed transfers of
care
- Promote investment in improved community care services aimed at
reducing the delay in transfers of
care
- Strengthen joint working between the parties to improve
discharge planning and encourage the timely provision of services
that enable discharge from hospital.
