Access to health records

As a patient you are able to access your medical records held by
the University Hospitals Bristol NHS Foundation Trust. To access
your medical records you should make a request to the Medical
Records Department for the hospitals you were a patient at or
the Patient Support and Complaints Team Office. The medical records
department will then send you a letter and a form to complete and
return.
£10 charge is made for this
service to cover administration costs and you will also be charged
up to a maximum of £50 for the copying of your medical records.
These charges are made in accordance with the Data Protection Act
1998. Once your form and payment have arrived at Medical
Records, they will begin the process, which may take a number of
weeks. We always aim to disclose your records to you within
the maximum deadline of 40 days from receiving your
application. In line with the Data Protection Act 1998, the
Medical Records Department will send your request to the
appropriate consultant for their written permission. The consultant
will decide on the best way for you to see your notes. This can be
done by making an appointment with the consultant, viewing your
medical records in the Medical Records Department or by photocopies
being sent to you directly.
